Surveillance Schedule Generator

The first three parameters in this schedule generator specify the range of thresholds on the cumulative risk of relapse. If you want to generate a schedule for just a single threshold, use the same threshold for the start and end of the threshold interval. The minimum interval between follow-up appointments sets a minimum number of months spacing between the dates in the surveillance schedule. Set it to 0 to have no minimum spacing.
